Multiple series line Chart issues

I have requirement to display the line Char week wise - error code id and already I have found out the week and I have data as below format

Here data 6/15/15 to 6/21/2015 having multiple error code count  so I want display the line chart as below format

I need to display the chart Week wise each error-code Count. how to do can you please suggest .
